State 2): H4v, line 5 begins "Church" in italic type, with a large, swash C; and K2v last line begins "Church of England ..." with the word "protestant" spelled correctly in the line above.
With a final advertisement leaf.
With a preliminary imprimatur leaf: "Z. Isham R. P. D. Hen. Episc. Lond. à sacris. 1685."
Signatures: A-F⁴ G⁴(-G4) H-I⁴ K².
