The 'Tractatus primus iohannis filii serapionis medici' in the translation by Gerard of Cremona, printed in 1479 and later, in seven books, each preceded by a table of chapters (ff. 1-116); followed by a list of synonyms, also printed in the editions of 1479 and 1530 (ff. 116-118v).
An index to the main text, tractatus 1-6, headed 'Tabula alphabetica', was added for William Horman on ff. vi-vii verso in the early 16th century. See N.R. Ker in The Library, 5th ser., 17 (1962), pp. 79-82.
Main text begins: 'Capitulum de caluicio primum de allopicia et tyria. [E]gritudines iste accidunt ...'
Folios v-vii, 119-122 are medieval flyleaves.
The 'Tractatus' and the 'sinonima' were foliated 1-118 around 1500; traces of an earlier foliation remain on ff. 4-21.

Collation: 1-14⁸ 15⁶.
Spaces for initials (4- or 5-line and 2-line) remain blank.
Secundo folio: huius egritudinis.
Rebound at Eton ca. 1720.
